You saw two ads in the same week. One promises to firm the lower face with focused ultrasound. The other promises exactly the same thing with radiofrequency. Neither one explains the difference, and you are the one who has to choose.
Here is that difference, in plain language. One thing up front, because it changes how you should read the rest: we do not offer HIFU. We work with radiofrequency, including XERF. So we have nothing to sell you on focused ultrasound, and nothing to hold against it either.
Both technologies start from the same idea. You heat the dermis, the deep layer where collagen and elastin live. Heat tightens the fibres already in place and pushes the skin to build new ones over the following months.
Up to that point, HIFU and radiofrequency tell the same story. What separates them is how that heat gets delivered, and everything else follows from it: what the session feels like, what kind of laxity it suits, how many appointments to plan for.
Tissue analysis after treatment has put the two approaches side by side. The finding is clear: these are not two versions of the same thing. They are two ways of placing energy into living tissue.
HIFU is focused ultrasound. Picture a magnifying glass concentrating sunlight onto a single spot. Hence the two words to keep if you keep nothing else. HIFU is focal: it hits hard, in one spot, at a set depth. Radiofrequency is diffuse: less intense, wider, over a longer time.
A grid of heat points and a warmed volume do not leave the same work behind them. A focal point creates a strong, local reaction, which the body then repairs by building tissue around it. A heated volume acts more evenly, through the full thickness of the treated area.
So what you end up with is not quite the same. The focal approach reads as a structural gesture, pinpointed and powerful. The diffuse approach reads as groundwork, which also improves suppleness and general skin quality along the way, because it does not stop at a point.
Neither approach is better in the abstract. They answer different situations, and it is the examination of your skin that says which one makes sense.

Pages comparing these two technologies argue about depths and mechanisms. In real life, what usually decides is how the session feels.
Concentrating a lot of energy on one point is something you feel. People who have had HIFU generally describe brief, hot, sometimes deep stings that repeat with every pulse. That is not abnormal and it is not a flaw in the device: it is the direct consequence of focusing. Some people tolerate it very well, others find the session long.
Spreading that same energy through a volume feels very different. Radiofrequency builds heat gradually, and most people describe it as a warm massage. Intensity is adjusted throughout the session, based on what you say you are feeling.
This deserves to be said plainly, because it often decides whether someone comes back. Firming is a long game. A session you dread is a session you end up postponing, then cancelling.
Here again, the physics explains the calendar.
HIFU delivers a lot of energy at once. So it is often presented as a one-time treatment, with a result that builds over the following months as collagen rebuilds.

You cannot remove excess skin, and you cannot reposition tissue that has descended.
So when laxity is marked, surgery stays the better answer. That is not pleasant to read and not pleasant to say, but it is the kind of thing you would rather hear before paying than after. The first is comfort, for everything said above. Firmness asks for consistency, and consistency asks for a session you do not brace yourself for.
The second is skin tone. Radiofrequency heats through tissue resistance; it targets no pigment. So it suits every complexion, without the risk of spots that some energies carry on darker skin. We get asked this often and the answer is simple.
The third is that it can be dialled. Not every skin has the same problem. XERF works deep on laxity of the whole face and the neck. TempSure is a gentler radiofrequency, zone by zone, for the mouth contour or a finer area. Two settings of the same physical principle, not two competing promises.
That is where most people look first, and it is also the most delicate area. Neck skin is thinner than facial skin, it moves constantly, and slackness under the chin can come from two very different things: skin that has lost its tone, or a small pad of submental fat.

The real deciding criterion is not "which is better." It is "which one matches what I have, and what I am willing to sit through." Both target mild to moderate laxity. Neither replaces a facelift.
