Who is responsible
9415-1867 Québec inc., doing business as La Clinique DC, 205, boulevard Curé-Labelle, Laval (Sainte-Rose) QC H7L 2Z9, is responsible for the personal information collected on this site.
For any question about your information, write to info@lacliniquedc.com or call 438 609-0094 and ask for the person responsible for the protection of personal information.
What we collect, and why
We only ask for what we need for the reason stated at the moment we ask for it.
- The guide and La DC Lettre: your first name and email, to send you the guide and then our emails, for as long as you stay subscribed.
- An appointment request: your name, email, phone number and the treatment you are interested in, so we can call you back and set the meeting.
- The contact form: your name, email and message, so we can reply.
- A conversation with Stella, our assistant: the text of your messages, to answer and, if you ask, pass your question to the team.
- Audience measurement: browsing statistics, only if you accepted cookies. Nothing runs before you agree.
What we do not do
- We do not sell or rent your information to anyone.
- We do not add you to any newsletter unless you asked for it by ticking the box provided.
- We never ask for your health insurance number or any health information by email or through the site.
Your consent
Every checkbox starts unticked and states what you are consenting to. We keep the exact wording you accepted and the time you accepted it: it is the only way to know later what you said yes to.
You can withdraw your consent at any time, without having to explain. An unsubscribe link is in every email, and you can reply STOP to a text message.
How long we keep them
- A subscription to La DC Lettre: as long as you are subscribed, then twenty-four months, so we can prove your consent if asked.
- An appointment request or a message: as long as it takes to handle it, then twenty-four months.
- A conversation with Stella: thirty days, then it is erased.
Who else has access
We entrust some tasks to suppliers, who may only use the information to provide us that service:
- GoHighLevel (United States): our customer relationship system, where subscriptions and appointment requests arrive.
- Vercel (United States): the hosting of the site.
- Stripe (United States): card verification when you book. We never see your card number: it goes straight to Stripe.
- Anthropic (United States): the language model that powers Stella.
Information that leaves Quebec
The suppliers above process data outside Quebec, mainly in the United States. Before entrusting anything to them, we verify that they offer adequate protection and that their contract requires them to use it only for us.
Stella, and automated decisions
Stella is a software assistant. She answers from the content of this site and makes no diagnosis: as soon as a question calls for clinical judgment, she hands over to the team. No decision about you is made automatically from your information.
Your rights
By writing to info@lacliniquedc.com, you can:
- find out what information we hold about you and get a copy of it;
- have it corrected if it is inaccurate or incomplete;
- withdraw your consent and unsubscribe;
- ask that it be erased, where nothing requires us to keep it;
- receive, in a common computer format, the information you provided to us.
Cookies
The site sets a technical cookie to remember your language and the state of your cart. Audience measurement only starts if you accept it in the banner shown on your first visit, and you can change your mind afterwards. The guide and the window offering it remember, in your browser only, that you have already seen them: none of that reaches us.
If an incident occurs
A confidentiality incident is an unauthorized access to, use or disclosure of your information, or its loss. We keep a register of them, as the law requires.
When an incident presents a risk of serious harm to you, we take steps to reduce its effects, we notify the Commission d’accès à l’information du Québec, and we notify you personally. Without waiting for the investigation to be over.
A complaint
Write to us first: we answer within thirty days. If our answer does not satisfy you, you may contact the Commission d’accès à l’information du Québec, cai.gouv.qc.ca.
