Hair Exosomes: How They Act on the Follicle

Exosomes are tiny cellular messengers. Here is a plain-language look at how they speak to your follicles, with no promises attached.

Exosomes are tiny vesicles, a kind of messenger your cells send one another to communicate. In hair treatment, we place them in the scalp to carry a regeneration signal to the follicle.

One thing to say up front: exosomes are not approved by Health Canada for hair regrowth, even though their use is common in clinics. What follows explains what is known about the mechanism, not a guaranteed result.

Understanding exosomes

What is an exosome?

In one sentence: an exosome is a vesicle, a tiny biological sac, measuring between 30 and 150 nanometres. About a thousand times smaller than the thickness of a hair.

Where they come from: every cell in the body produces them naturally. They are how cells communicate, the letters they send each other to exchange information.

What they contain:

  • Growth factors, the regeneration orders
  • Repair proteins, the rebuilding tools
  • Messenger RNA, the instructions
  • Nourishing lipids, the cellular nutrients

Exosomes in aesthetic medicine

Where the ones we use come from: for hair treatments, exosomes come from controlled cell cultures in a laboratory, most often of plant origin. Permitted sources vary from one country to another, and it is a perfectly normal question to ask at your consultation.

Why those sources: these cells produce exosomes in consistent quantity and quality, rich in growth factors useful to the follicle.

How they act on your hair

Think of exosomes as small messengers whispering to the hair bulb: "Wake up, it's time to grow."

Here is how that conversation goes.

1. Placement

What happens: exosomes are placed directly in the scalp, through micro-injections with a very fine needle or through microneedling, depending on the protocol.

Why that route:

  • Precise targeting: they arrive exactly at follicle depth
  • Nothing is lost on the way, unlike a surface application
  • Action starts immediately, within minutes

2. Fusion with the cell

What happens at the microscopic scale: once placed, exosomes fuse with the membrane of your follicular cells, like a letter slipped under a door.

The content of the message:

  • Wake your dormant stem cells
  • Repair the accumulated damage
  • Increase your keratin production
  • Improve local circulation

3. The cell's response

Reactivating dormant follicles: follicles in the resting phase (telogen) get the signal to return to the growth phase (anagen).

Strengthening weakened follicles: miniaturized follicles can regain calibre.

Action on the hair cycle

A longer anagen phase: exosomes encourage the hair to stay in growth longer, which means longer, stronger hair.

A shorter telogen phase: they reduce time spent resting, so turnover speeds up.

A cleaner catagen phase: the transition between growth and rest happens more smoothly, and fewer hairs fall prematurely.

Chronic scalp inflammation is one of the causes of hair loss. It smothers the follicle slowly, without anyone noticing.

Exosomes and minoxidil: opposite logics

That is why we do not point anyone toward minoxidil here: we would rather bet on your own follicles, early, than on a product you apply daily for life.

Exosomes:

  • Improve overall quality
  • Work on the follicles still there
  • Slow the loss to come

If you have bare areas, a transplant is the right answer and we will tell you so plainly.

What you can expect

The markers below describe a typical progression, not a promise: response varies from person to person, and nobody can guarantee you a result.

Around 1 month:

  • Daily shedding drops
  • Hair is shinier and more resilient
  • Texture improves

Around 3 months:

  • Visible regrowth in thinning areas
  • Fine hair thickens
  • More density at the roots

Around 6 months:

  • A clear change across the whole head of hair
  • Fuller hair
  • Previously thin areas filling in

Around 12 months and beyond:

  • Results settle
  • The hair cycle is rebalanced
  • A brake on further loss

The types of loss concerned

Androgenetic alopecia: slower miniaturization, reawakening of the follicles still present, general gain in density.

Diffuse loss in women: strengthening of fragile hair and restoration of overall volume. In women, we look first at iron, thyroid and hormones: restarting a follicle without correcting the cause is rowing upstream.

Telogen effluvium: faster recovery and a shorter shedding phase.

Alopecia areata: this is an autoimmune condition, followed in dermatology. We do not treat it here, and we will refer you.

Who it is for

The right profile

  • Recent or moderate loss
  • Follicles still present, but weakened
  • A wish for something that works with your biology
  • A contraindication to PRP

Three situations where exosomes have the edge:

1. When plasma quality would be compromised: smoking, chronic stress, disrupted sleep, an unbalanced diet.

2. When health gets in the way: poorly controlled diabetes, anticoagulants, certain chronic conditions.

3. When you want to avoid the blood draw: some people simply do not want one, and that is reason enough.

When to prefer exosomes over PRP

If your plasma is less rich: lifestyle and health directly influence PRP quality. Exosomes always arrive at the same concentration.

If you want a high, consistent concentration: the one in exosomes generally exceeds what a standard PRP delivers.

For practical reasons: no blood draw, a shorter session, fewer variables to control.

It is a young field, and it moves fast. Three directions are taking shape: compositions tailored to each person's profile, new sources of exosomes, and new ways to deliver them, from automated microneedling to slow-release devices.

We follow them closely without announcing them early: a research avenue is not a treatment.
